How do I register my child?

WHERE TO REGISTER:
Students are to be registered at the
school they will attend in September 2006.
THE FOLLOWING WILL ASSIST YOU IN
REGISTERING YOUR CHILD:
 | A parent must accompany the child to
be registered. |
 | A birth certificate or hospital birth
registration certificate must be presented at registration. |
 | Kindergarten entrants must be 5 years
old on or before September 1, 2006 |
 | Child's social security number. |
 | Written physician's verification
showing dates of all immunizations is required at the time of
registration. |
 | Parents or guardians must present
acceptable documents to prove their residency in the Neshaminy
School District. The following documents will be considered:
dated and signed deeds,
settlement papers or rental contracts, current tax statements
or bill, current address on driver's license, current utility
bill or other legal documents indicating permanent address.
Proof of residency, shall be decided by the principal on the
documents submitted. |
Students registering for Grade 1 must be 6 on
or before September 1.
Proper immunization records are as follows:
* Four (4) doses of Diphtheria and Tetanus Vaccine with at least one of those doses on or after the child's 4th birthday.
*Three (3) doses of Live Oral Polio Vaccine or 4 doses of
Salk (injectable) - vaccine.
*Three (3) doses of Hepatitis B Vaccine
*Two (2) doses of Live Measles Vaccine given on or after
the first birthday or a blood test showing antibody protection.
*One (1) dose of Live Mumps Vaccine on or after the first
birthday or a signed physician's statement indicating your child
had mumps disease.
*One (1) dose Live of Rubella Vaccine after the first
birthday or a blood test showing antibody protection.
*Proof
of varicella (chicken pox) immunity either by vaccine, history
of disease, or lab test.
In accordance with Pennsylvania law, children
will be barred from entering school in September 2006, unless at least
one dose of each antigen is given.
SPECIAL NOTES REGARDING IMMUNIZATIONS
Strict exceptions to the listed
immunizations are provided only on medical, religious or moral
grounds. Consult the office of Pupil Services (752-6395) or
your school principal for details on procedures for exceptions.
Parents and guardians are encouraged
to keep official physician's records of their child's
immunizations. In order to protect the health and well-being
of all children, immunizations are required.
All of the school districts in
Pennsylvania must comply with immunization laws. Virtually
all the states of the United Stated have laws which meet or exceed
the Pennsylvania requirements.
